Our office must receive a completed Application for Assistance, View exit disclaimer policy page for links to third-party websites. including:
Proof of monthly income
Social Security numbers for all household members
Proof of pregnancy (if applicable)
Proof of residence
Valid Colorado ID
Proof of lawful presence and a signed affidavit for applicants 18 years and older
Once the application is received, an appointment is scheduled for an interview with an Intake Program Specialist to determine eligibility.
Applicants are assessed during the interview and a determination is made on what supportive services will best assist the family in reaching their self sufficiency goals.
The applicant may qualify for a State or County Diversion which assists in meeting immediate needs.
